A Warrington plant hire firm has been fined £200,000 after one of its employees was killed at work.
The 55-year-old worker died after falling five metres on August 16, 2006.
An investigation into the tragedy by the Health and Safety Executive, (HSE) showed that the company failed to follow its own health and safety guidelines for working at height.
The Kent-based man had worked as a fitter at the company's depot at Tovil, Maidstone, for less than three months when he died in the work accident.
At the time of his death he was preparing accommodation units including site huts, welfare units and storage containers. The court heard how the worker climbed on top of two of the units which had been stacked on top of each other to attach lifting chains so that the top unit could be lifted down.
The HSE's investigation found that workers at the depot had not been issued with safety harnesses or been trained to use them. Most employees also did not realise there had been a special procedure for this kind of work to prevent construction site accidents such as falls.
