A carpenter working on the construction of Heathrow's Terminal Five died in an accident at work when he fell 50 feet to his death as a concrete slab on which he was standing collapsed.
The two construction companies involved in the supply and use of the sub-standard threaded shoring adapters (TSA) which failed, SGB Services Ltd and Laing O'Rourke, were fined a total of £105,000 for breaches of health and safety regulations and an equal amount of costs.
An engineer's assistant, who also fell in the building site accident and suffered head, leg and back injuries, has received £270,000 personal injury compensation.
The judge at Isleworth Crown Court described the incident as a tragic accident and said the companies failed to have the necessary quality control checks in place to remove the defective TSAs which were not capable of carrying the required load.
Laing O'Rourke, which settled all civil damages claims over the work accident, has co-operated with the Heath and Safety Executive (HSE) and introduced new systems of training, control and risk assessment to avoid further construction accidents.
